Diagnostic imaging can sometimes reveal interesting shapes. In this case we describe a transoesophageal echocardiogram of a mitral valve prolapse in shape of a heart sign, which was successfully repaired.

Investigation for mitral valve disease can provide a number of exciting and unexpected images. Here, we present an image from the intra-operative TOE mid-oesophageal four-chamber view where the prolapsing A3, P3, and the commissural part of the valve resembled a diagram of heart, giving the impression of Heart in the Heart. The repair of the defect was achieved by neocords and a ring, and produced excellent results (see Supplementary data online movie 1).
